Your Personal Information
Your personally identifiable information is held in strict confidence. “Personally Identifiable Information” refers to any information that identifies or can be used to identify, contact, or locate the person to whom such information pertains. This includes information that is used in a way that is personally identifiable, including linking it with identifiable information from other sources, or from which other personally identifiable information can easily be derived, including, but not limited to, name, address, phone number, fax number, email address, financial profiles, social security number, and credit card information. To the extent unique information (which by itself is not Personally Identifiable Information) such as a personal profile, unique identifier, biometric information, and IP address is associated with Personally Identifiable Information, then such unique information will also be considered Personally Identifiable Information. Collection and Use of Information
In order to utilize services provided by OnlineCouch.com you must complete an intake form where certain fields are required prior to performing services. Your name, address, telephone, email, and a brief statement of your problems will be required information during the intake process. Your personal information will only be used as necessary to contact you if you are a registered customer with pending sessions or post-session issues. The information required is necessary to ensure accuracy of billing and your email address will only be used for customer contact when a session has been scheduled. Email addresses may be used for OnlineCouch.com marketing and mailing lists, but only if the user has elected such services. We do not sell, rent or release any information gathered on our site to third parties. All information gathered will be used only for internal marketing analysis and to improve our products, services and websites. You will have a choice to opt-out of solicited emails during the registration/intake process. All other information requested during the intake process is strictly optional.
Some clients may wish to remain anonymous, that is, have treatment without the therapist knowing their name. Such requests are problematic and if a therapist agrees to such an arrangement, the agreement can only be tentative and conditional. Therapists have a right, and in some situations, an obligation to know who their clients are. One reason for this is the issue of “dual relationships,” the other is compliance with the law regarding risks to or presented by clients. For this reason we require personally identifiable information prior to issuance of services.
To maintain anonymity, you may elect to enter false information in the required at your own discretion. Although, Colorado Couples Counseling and Sex Therapy, LLC, OnlineCouch.com and your therapist cannot be held liable under such circumstances.
Our information systems keep a record of the secure chat between visitor and clinician. The content of the online consultation is securely retained for legal and administrative purposes only and will not be used for any other purpose other than a recorded record of conversations.
Customer Tracking Information
We collect information about our visitors for hit tracking and to help diagnose problems with access from differing sources and browsers. At a minimum, your IP address, browser version, and referrer will be collected during your initial visit. Your IP address may be used to provide demographic statistics and will not be shared at any time. Tracking information is not tied to your personally identifiable information and is thus considered anonymous information, to be used for informational purposes only.
Customer Information Self-Administration
Users of OnlineCouch.com will be provided with a mechanism to correct, access and update pertinent Personally Identifiable Information entered through the site. Our security measures are in place to protect loss, misuse, unauthorized access or disclosure, alteration, or destruction of all information collected during your visit to OnlineCouch.com. All information requested during the intake process and all chat and email sessions will be transmitted utilizing 128bit Secure Sockets Layer (SSL) encrypted communications and stored in a secure database. All communications are recorded for security purposes and to serve as session documentation for the therapist. Communications are always private between you and your therapist only. Your therapist is located in a secure, private suite where the ability to overhear telephone, voice chat, or in-office conversations has been restricted. Users are automatically logged off of their accounts after a timeout period has been reached.
WE USE APPROPRIATE SECURITY SAFEGUARDS.
United States law requires that any suspicion of risk of child or elder abuse be reported to welfare authorities or law enforcement authorities. If a professional believes that a child or elder is at risk of being abused, has already been abused, or that someone who previously abused children or elders is still a threat, they will often make an effort to report it to social services agencies and/or the police.
Most United States courts have the power to subpoena our records and a professional’s records if you are involved in a court case in which your relationship with Colorado Couples Counseling and Sex Therapy, LLC (OnlineCouch.com), or the professional is somehow relevant. Although this may be rare, if you have any questions, it is better to contact your lawyer prior to disclosing any information you may deem private.
If someone’s life is in danger, the professional may choose to report it. This applies to potential cases of suicide or homicide.
If clients themselves request information about their treatment or their treatment records, or give consent to the therapist to release that information to others, then therapists generally must comply with their clients’ wishes.

By using this service you are responsible for getting the proper help that you need and OnlineCouch.com has no legal bindings or ties to assist you in getting help.
Your credit or check card is processed directly through OnlineCouch.com, which uses a third-party verification and billing system in order to bill the appropriate charge to your account. The only information shared with this third-party billing system is your name, account number, expiration date on the card, and charge amount. Neither OnlineCouch.com nor Itransact.com will use this information for any purpose other than that of processing these transactions. All charges will appear on your statement as Colorado Couples Counseling and Sex Therapy, LLC.
Notification of Changes
